The visitation checklist documents your attendance and the time you spent at the host school. The cooperating teacher (mentor column) must initial your attendance each day you visit the school. You are required to utilize the following time log for documentation of your visits to the school during the 10 hour requirement.  At the conclusion of the field experience, you will complete the Time Log Summary and have the cooperating teacher verify and sign.  These documents are uploaded into Tk20 along with the other requirements of the short field experience.

Final Products from the Short Field Experience might include:

1.     Completed visitation check list, initialed and signed by the mentor teacher
2.     Activities log, initialed and signed by the mentor and candidate
3.     Reflective summaries of the observations and the corresponding observation / evidence forms. A minimum of five (5) observations and five corresponding reflections are required
4.     Documentation of four (3) required interviews and three corresponding reflective interview summaries.
5.     Final Report (3 pages). This report provides an overview of the field experience and summarizes what the candidate learned from engagement in professional duties, observations, interviews, discussions and other interactions with the students and educators at the host school. Discussions should focus on measures to accommodate student needs and to ensure all learners are provided provisions that facilitate the acquisition and comprehension of relevant information
6.     A final evaluation from your mentor teacher will be completed within the Tk20 electronic system indicating your level of performance within your assignment

The final report provides an overview of your professional short field experience and is included in the final submission. This report should be a three-page formal paper, double-spaced, and typed (APA format). In this summation you are to:

1.     Provide a description of the physical facility and the available supplies and/or equipment in the school to which you were assigned;
2.     Provide a general description of the component of the N C Standard Course of Study that was taught during your visits;
3.     Provide a description of instructional efforts to facilitate learning for exceptional students;
4.     Discuss the provisions to infuse technology and other resource in the learning activities;
5.     Describe your overall impression of the characteristics and work of the teacher;
6.     Discuss venues to provide appropriate learning experiences for all learners and how you as a professional educator will work toward implementing these measures;
7.     Discuss your feelings toward your goal of teaching following the completion of your field experience;
8.     Provide reflections about the field experience that will influence the teacher that you wish to become in the future.
